Estoy tratando de hacer un formulario, pero no me funciona

Este es el código:

Me funciona Container, Inventory, Order2 y Suborder pero Bill of Lading no. Me podrías ayudar con el problema. Soy nuevo en esto. De antemano muchas gracias!!!
